Deep Life's Open Revolution Safety Initiative in life support systems

Deep Life develops life support systems and other safety products. It launched its Open Revolution safety initiative in 2005, to ensure the highest standard of safety in its products, and to disseminate information to raise the general level of safety awareness in the industry.

To carry the Open Revolution trademark, a life support system must be proven publically to achieve 1 billion hours Mean Time Between Critical Failure. This requires:

1. Full disclosure of the FMECA
2. Full disclosure of the operation of the safety systems , including their verification
3. Shown to protect the user in all credible failure scenarios, including a list of conditions that have caused near misses and fatal incidents reported on rebreathers.
4. Open publication of the calculation showing at least a billion hours between Critical Failure.

The safety data behind Deep Life's rebreathers is contained in the following documents.

	Formal (maths) models Maths models for the rebreather, its environment, and decompression algorithms. These are in Matlab and Simulink for ease of use, and for Monte Carlo testing of independently written code.
	Accident Database and Reviews The Comprehensive Database of Rebreather Fatal Accidents, and incident analysis.
	Design Validation Reports These are the detailed test and validation reports that identify the limits of each subassembly.
	HAZOPS Formal HAZOP and safety review reports. Note, the HAZID review forms Volume 6 of the FMECA.
	FMECA There are 7 volumes in total covering the whole design, from top down and from bottom up, for the electronics, software and mechanics, and a Fault Tree Analysis.
	Compliance Reports These are the detailed test reports showing compliance with the appropriate standards. The page will be populated as compliance reports are audited.
